I authorize Compass Management Group, Inc. (Manager) to initiate to the account listed above at the Financial Institution listed above and, if necessary, initiate adjustments for any transactions credited/debited in error. This authority will remain in effect until I notify Manager in writing to cancel it in such time as to afford Manager and Financial Institution a reasonable opportunity to act. 

Withdrawal will occur on the fifth (5th) day of the month in which the payment is due, or closest business day. Withdrawal will include annual/quarterly/monthly assessment (Association Dues), as well as any late fees, fines, chargebacks, etc., if applicable.
